Athletic Clearance for Participation

Step 1: Register Online through FinalForms

Additional forms may be required to be submitted based on your situation (e.g., Running Start, New Student packet, etc.)

Step 2: Maintain a Current/Valid Sports Physical on File with the Athletic Office

Physicals are valid up to 24 months from the date of the examination
A printable Sports Physical Form can be found here and brought to your Medical Provider for completion
To update a Sports Physical in our system you can upload the document in FinalForms, Fax to 425-366-5702, Email, or turn in a hard copy to the Mariner Main Office or Athletic Office

Step 3: Obtain an ASB Card

Students can purchase an ASB card for $40 on campus at our Bookkeeping Offices or Online
Fee waivers will be granted to students who qualify for free or reduced-price meal benefits and have consented to share their data with other programs within the District.
