Step 1: Register Online through FinalForms

Additional forms may be required to be submitted based on your situation (e.g., Running Start, New Student packet, etc.)

Step 2: Maintain a Current/Valid Sports Physical on File with the Athletic Office

Physicals are valid up to 24 months from the date of the examination
A printable Sports Physical Form can be found here and brought to your Medical Provider for completion
To update a Sports Physical in our system you can upload the document in FinalForms, Fax to 425-366-5702, Email, or turn in a hard copy to the Mariner Main Office or Athletic Office

Step 3: Obtain an ASB Card

Students can purchase an ASB card for $40 on campus at our Bookkeeping Offices or Online
Fee waivers will be granted to students who qualify for free or reduced-price meal benefits and have consented to share their data with other programs within the District. More information, the Family Income Survey and the Consent Form in ParentSquare can be found on our Nutrition Services page: Mukilteo School District - Free and Reduced-Price Meals (mukilteoschools.org)
